PHOENIX, Ariz. -- Anthony Lee Cunningham, 40, of Gilbert, Ariz., passed away Monday, Feb. 6, 2006 at Banner Medical Center in Arizona.

He was born March 31, 1965.

Survivors include: his mother Rose L. Schubla of Linton; his friend Melvin L. Kendall of Arizona; his sister Geraldine Fay Watts of Washington N.C.; and family friend Greg Spark.

He was preceded in death by a sister Valerie Rea Cunningham on Dec. 6, 1966, a brother Berle Thomas Heaton on Oct. 16, 2001, and his step-father James Schubla on July 24, 2001.

His cremation took place Feb. 7, 2006 in Phoenix, Ariz., at Greenwood Memory Lawn Crematory.
